1. Please ensure that your claim should consist of the following documents:-
  • The original signed and attested Claim Form (Statutory Declaration);
  • A brief written narrative of how you were introduced to the scheme and your investment history;
  • A copy of your NRIC; and
  • All relevant supporting documents (Example: proof of payment such as bank deposit slips, credit card payment slips, proof of telegraphic transfer via internet banking or ATM)
2. The Claim Form (Statutory Declaration) should be filled up in either Bahasa Malaysia or English.
3. Please submit your Claim Form (Statutory Declaration) together with all supporting documents to Investor Affairs and Complaints Department, Securities Commission Malaysia by 5pm, 14 March 2016.
4. Securities Commission Malaysia shall not entertain claims submitted after the deadline or submissions which are not supported by relevant documents or submissions in any language other than Bahasa Malaysia or English.
5. Securities Commission Malaysia shall have the absolute discretion to approve or reject the submitted claims.
